How is extra fuel added during a fusion reaction (artificial '' not the sun'')

Question

I wonder because these reactions produce alot of heat (and the threshold to start the reaction is very high) and from what I understand they try to limit the heat transfer by having as much of an isolator or vacuum between the centre as possible. Or is there a connection to the middle and if so doesn't that cause heat transfer problems?
